Last year 3 Quebec worker's died  due to lack of communication.  While performing repairs on a tank in a confined space, an employee loses consciousness.  A second coworker attempts to provide assistance, but also lost consciousness.   A third employee trained in first aid also succumbed to the oxygen - deficient atmosphere. The coroner's report reveled poor communication, lack of training and lack of information on the risk of confined spaces were prevalent.
